Intra - Osseous Venography in Patellofemoral Disorders

چکیده

Patients with severe degenerative osteoarthritis of the hip and knee have impaired venous drainage from the juxtachondral cancellous bone across the cortex (M#{233}riel et a!. 1955; Phillips 1966; Arlet, Ficat and Sebbag 1 968; Arnoldi, Linderholm and MUssbichler 1 972). Intra-osseous phlebography will demonstrate the absence of the normal drainage pattern, with delayed emptying of the contrast medium from the medullary cavity. Brookes and Helal (1968) found that in rats increased trabeculation of bone and calcification of the articular cartilage appeared as a consequence of chronic venous congestion, and suggested that this might be the precursor of osteoarthritis. Primary or secondary venous engorgement and intra-osseous hypertension play a significant role in the aetiology of pain in osteoarthritis. Arnoldi, Lemperg and Linderholm (1971) demonstrated a reduction in intra-osseous pressure following osteotomy of the femur, and Morscher (1978) noted that pain was produced when intra-osseous phlebography was performed under local anaesthesia. We have investigated the state of the venous drainage of the patella in patients with chondromalacia patellae and patellofemoral osteoarthritis. We agree with the view of Shoji, Sakai and Granda (1 974) and of Mankin (1 976) that chondromalacia patellae is an early stage in the development of patellofemoral arthritis.

Anatomy and surface geometry of the patellofemoral joint in the axial plane.

We studied the anatomy of the patellofemoral joint in the axial plane on cryosections from a cadaver knee and on MR arthrotomograms from 30 patients. The cryosections revealed differences in the geometry and anatomy of the surface of the articular cartilage and corresponding subchondral osseous contours of the patellofemoral joint. On the MR arthrotomograms the surface geometry of the cartilage...

Modified Elmslie-Trillat Procedure for Distal Realignment of Patella Tendon

Patellofemoral dysfunction, due to either a patellofemoral malalignment or patellar instability, is a complex and debilitating condition that significantly decreases the knee function. Conservative management may yield significant clinical outcomes; however, when morphologic anomalies are identified, the surgical approach should be employed. Hence, several surgical procedures have been describe...

Magnetic resonance imaging of the patella: a comparison of the morphology of the patella in normal and dysplastic knees.

We retrospectively analysed the MR scans of 25 patients with patellofemoral dysplasia and ten control subjects, to assess whether there was any change in the morphology of the patella along its vertical length. Ratios were calculated comparing the size of the cartilaginous and subchondral osseous surfaces of the lateral and medial facets. We also classified the morphology using the scoring syst...

A 25-year retrospective epidemiological study of intra-osseous lesions of jaw bones in Isfahan population, Iran

BACKGROUND AND AIM: Intra-osseous pathologic lesions of the jaw bones have different radiological and clinicopathological features from benign to malignant lesions. The aim of this study was to determine the prevalence of jaw lesions in Isfahan, Iran, during 1992-2017. METHODS: In this descriptive-analytical study, all re...
